What are the responsibilities and job description for the Support Coordinator position at Utah Case Management?
Utah Case Management is looking to hire an individual with experience as a Support Coordinator for the Division of Services for People with Disabilities (DSPD). The Support Coordinator will provide and support coordination services such as coordinating care, assessing clients' needs, communicating with service providers, etc. Most tasks can be completed remotely besides when doing in-person monthly visits with clients.
- Time Requirement: The time requirement would start less than 15 hours per week, but could grow as the Support Coordinator gains more clients on their caseload. The individuals’ client caseload would start out as a slow build. The building of a caseload is based on the Support Coordinator and how many clients they would like on their caseload.
- Pay: Pay is based on the size of the Support Coordinator's caseload. Support Coordinator will be paid a set rate per client on their caseload per month. There is paid training.
- Location: We are hiring an individual in Utah County. The individual must be a resident of the Utah County area and willing to travel to visit clients. Travel may be required to go outside Utah County boundaries.
Qualifications:
- Must have a Bachelor's Degree in the field of human services or related field and/or Registered Nurse License
- Must have one year experience working with individuals with intellectual disabilities.
- Experience working with the Division of Services for People with Disabilities (DSPD) (preferred).
- Passionate about working with individuals with intellectual disabilities
- Must have interpersonal skills
