Coordinates services as directed by attending or referring physician or clinical case manager. Oversees delivery of individualized services to patients and families.This position is responsible for the coordination of care activities under the direction of a clinic manager. Interacts with representatives from insurance companies, health maintenance organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies. Will assist patients with housing, food security, finances, and transportation when needed, along with helping patients address other social determinants of health. This position provides exceptional care for our patients and guests by offering them professional service and creating a memorable patient experience. The incumbent accurately and professionally handles clinic operations tasks including, but not limited to, answering phones, patient registration and scheduling processes.
