Summary:
In a supportive and patient-centered manner, guide patients though the psychosocial assessment process shortly following admission to the hospital. Enter a thorough and concisely-written assessment into the EMR. Complete other assessment measures as indicated (e.g., PCL-5, BDI, BAI, etc.) at admission and/or throughout the treatment episode. Adhere to all hospital policies and procedures regarding quality assurance, safety, environmental and infection control and all regulatory and accreditation requirements.
Essential Duties and Responsibilities Include the Following:
1. Complete thorough psychosocial assessments,
2. Complete other assessments as indicated,
3. Participate meaningfully in multi-disciplinary treatment team meetings, concisely and clearly presenting patient information and conceptualization,
4. Complete all required paperwork in agency’s EMR meeting all regulatory, UR, and quality assurance standards,
5. Work collaboratively with all departments and teammates, communicate pertinent patient information to other staff/departments as necessary,
6. Provide crisis intervention, as needed,
7. Attend trainings and other meetings as required.
Education:
Master’s degree from an accredited institution with a major in chemical dependency, psychology, social work, counseling, nursing (with a clinical specialty in the human services) or other related field required.
- Master’s degree or above - a practicum in a drug & alcohol setting.
- Drug & Alcohol Certification (AAC, CAAC, CADC, CAADC) as granted by the Pennsylvania Certification Board preferred.
- Professional License through the Department of State in a clinical field and/or credentialing through PCB preferred.
