What You Will Do:

Facilitates safe and timely discharge of patients requiring post acute care services, or special transportation in coordination with care management team. Coordinates the legal process and paperwork involved with: involuntary patient admissions, protective services, guardianships and adoptions.

What You Will Bring:

Education
MSW from an accredited social work program or equivalent education/experience

Experience
Previous social work experience in a medical setting desired.

Certification & Licensures
BLS Certification (Basic Life Support) - American Heart ‘Healthcare Provider’ (HCP) - AHA approved required *
* New hires must have American Heart Association (AHA) appropriate certification prior to completion of orientation.

Qualifications
Knowledgeable in stages of human growth and development for neonate, pediatric, adult and geriatric populations. Skills in interpersonal relationships, clinical assessment, group process and high levels of verbal and written communication. Ability to interact with other professionals as part of a multidisciplinary team.
