We are currently looking for a qualified applicant to fill the following position: FT - Case Aid - ($16.25hr - $19.25hr) & $1500 Sign-on bonus! $750 at the start and $750 at 6 months!
Job Description: The Case Aid/Transporter is responsible for providing transportation to youth, menu planning and food purchasing, and coordinating the maintenance of the site. The Case Aid/Transporter is under the authority of the regional Sr. Vice-President and reports directly to the Program Supervisor.
Essential Functions:
Provide safe transportation for youth to scheduled medical, therapy or other associated appointments and family visitations. Document all trip information per protocol.- Support the Life Skill Case Manager with day to day functions, and be a role model for youth who are learning household and daily living skills
- Organize and prepare weekly meal plans that are reviewed and shared with the young adults.
- Supervise and assure safety of residents, and provide day coverage as needed or assigned at the residential homes.
- Represent the program and agency in a professional manner and maintain positive community relationships in a proactive manner.
- Address and report any crisis or critical situation as defined by agency and/or county, state or federal regulations according to the agency's formalized practice and protocols.
- Understand and adhere to Valley Youth House policies, practices and procedures.
- To participate in weekly scheduled supervision with direct supervisor and be prepared with an agenda.
- To attend and actively participate in team meetings.
- To attend both external and internal agency trainings and workshops and complete and pass all required trainings as set forth in the funder and licensing regulations as required.
- To complete other related tasks as appropriate or assigned.
- An individual 21 years of age or older, with the ability to identify with and adapt to the goals and philosophies of the independent living program. Candidates with lived experience may be considered..
- Possess sound judgment and knowledge of adolescent development and behavior.
- Display organizational skills and highly adept at managing multiple tasks and assignments.
- Successful team members are creative thinkers, are flexible in their approaches to people and projects, and are capable of working with diverse groups of the population.
- Must have and maintain a valid driver's license including vehicle insurance and inspection on any vehicle used for daily work and transportation of clients.
- Must receive a passing score on annual motor vehicle clearance to be eligible to operate agency vehicles.
- Ability to navigate stairs is highly preferred (as some locations are not always handicap accessible).
- Proficient in computer knowledge & skills (i.e. Microsoft Windows software); familiarity with the operations of external databases and Google Suite is desired.
- Never have been convicted of any prohibited criminal or child abuse offense.
- Minimum of high school diploma or GED required, combined with one-year of experience working with adolescents is preferred, lived experience will be considered.
