Job Description:
As a Rehab RN, you will work closely with a multidisciplinary team to provide comprehensive care to patients undergoing rehabilitation. Your role will encompass both medical and supportive aspects, aiming to help patients regain physical, cognitive, emotional, and social functions. You will work with patients across a wide range of age groups and conditions, including but not limited to:
Responsibilities:
Assessment and Planning:Perform initial and ongoing assessments of patients' physical, emotional, and cognitive status to develop personalized rehabilitation plans.
Collaborate with physicians, therapists, social workers, and other healthcare professionals to determine appropriate goals and interventions.
Medication Management:Administer medications as prescribed by physicians and monitor patients for any adverse reactions or changes in condition.
Patient Care:Provide direct care to patients, including assistance with activities of daily living (ADLs) such as bathing, grooming, dressing, and feeding.
Assist patients with mobility exercises, transfers, and ambulation to promote physical recovery.
Monitoring and Documentation:Regularly assess and document patients' progress towards their rehabilitation goals.
Maintain accurate and thorough medical records, including treatment plans, interventions, and patient responses.
Education and Support:Educate patients and their families about the rehabilitation process, home care instructions, and strategies for maintaining progress outside of the clinical setting.
Offer emotional support and encouragement to patients as they work towards recovery.
Collaboration:Collaborate with physical therapists, occupational therapists, speech-language pathologists, and other specialists to coordinate and implement a holistic rehabilitation plan.
Patient Advocacy:Advocate for the needs and preferences of patients, ensuring that their care aligns with their values and goals.
Discharge Planning:Participate in discharge planning by coordinating post-rehabilitation care and services to ensure a smooth transition from the clinical setting to the patient's home or another care facility.
Continuous Learning:Stay up-to-date with advancements in rehabilitation techniques, nursing practices, and healthcare technologies.
Qualifications:
- Active and valid RN license.
- Graduation from an accredited nursing program.
- Strong interpersonal and communication skills.
- Empathy and patience when working with patients who may be facing challenges.
- Ability to work effectively in a team-based environment.
- Strong critical thinking and problem-solving abilities.
- Familiarity with rehabilitation principles and practices.
Job Types: Temporary, Contract
Pay: $45.00 - $50.00 per hour

Weekly schedule:
- 5x8
- Monday to Friday
- Rotating weekends
Ability to commute/relocate:
- Jersey Shore, PA 17740: Reliably commute or planning to relocate before starting work (Required)
License/Certification:
- BLS Certification (Required)
- RN License (Required)
Job-related location requirement:
- Candidates must be within a 25-mile radius.
