Medical Coordinator

Responsibilities may include:
- Managing and coordinating medical services for individuals served.
- Assuring all appointments, follow up, discharge, and training needs are developed and implemented.
- Ensuring that individuals served are provided with daily health care needs, medication administration, doctor visits, etc.

Requirements for this position include:
- At least 18 years of age.
- High school diploma or GED.
- Personal, reliable vehicle that you're willing and able to use to transport individuals served.
- Valid driver's license and car insurance.
- Fewer than 6 points in the last two years on your motor vehicle record.
- Can push, lift or carry up to 50 lbs. and can climb, balance, kneel and crouch.
