THE OPPORTUNITY
The Medical Assistant is responsible for using our software to perform Chronic Care Management for chronically ill patients. Perform routine administrative and patient care supportive duties in the medical practice to assist medical and/or nursing staff in the examination and treatment of patients and ensure the office runs smoothly.
KEY RESPONSIBILITIES
- Complete patient care plans and perform CCM via the CCM software and provide feedback to the Clinical Staff Manager on an ongoing basis.
- Frequent contact with patients to support/manage the Chronic Care Management program to increase positive outcomes.
- Constant communication to and from Primary Care Clinician or designee regarding patient emergent needs and/or life-threatening episodes.
- Document all client communications (verbal or written) accurately.
- Demonstrate ability to work with various cross-organizational areas to meet the needs of CCM's clients.
- The expectation that you will become skilled at using technology including the CCM platform, secure email, etc.
- Excellent customer service skills demonstrated by positive feedback from customers.
- Keep your clinical staff manager informed of all issues pertinent to the care plan process and any known or perceived issues.
- Contribute as a positive member of the Department by supporting all members of the team in a productive and constructive manner.
- Adherence to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications.
EDUCATION AND EXPERIENCE
- Medical Assistant Certified
- Bilingual is preferred, but not required.
- Experience working with Electronic Medical Records (EMR) software as a user is preferred.
- Great communication skills
- Ability to build relationships and quickly develop trust with patients and client medical practices.
- The ability to thrive in a fast-paced environment.
- Highly motivated, results-oriented with strong skills in presenting, communicating, organizing, multi-tasking, and project development.
- Solid problem-solving and consultative skills
- Excellent computer skills
- Proficient with Outlook
