Virginia Health Services is hiring a RN Resident Assessment Coordinator! This is a Full Time Position work Mon - Fri, 8am - 4:30pm. Location could vary between Hampton, Newport News & Yorktown, VA.

The RN Resident Assessment Coordinator has the following responsibilities:
- Completing accurate and timely MDSs, CAAs and care plans in accordance with the RAI guidelines for all residents in the facility.
- Completing required documentation in an accurate and timely manner.
- Providing resident status updates as needed to appropriate personnel.
- Communicating with unit nursing staff regarding each resident's plan of care.
- Attending and participating in department/facility meetings as scheduled.
- Assisting with reviews of all pre-admission medical records. Providing accurate diagnosis codes and assuming the role of the managed care coordinator in their absence.
- Reviewing, creating and transmitting MDSs.
- Will have on call responsibilities.
- Adhering to facility policies and procedures and participants in facility quality improvement and safety programs.
- Promoting and demonstrating positive customer service towards VHS, co-workers, residents and visitors.
Requirements:
- Graduate of an accredited School of Nursing and holds current Virginia RN license.
- Experience/education in Gerontological Nursing required.
- RAC certification preferred.
- Computer knowledge preferred.
- Ability to read, write, analyze, and interpret information.
- While performing the essential functions of this job, the employee is regularly required to walk, turn, bend, stoop, lift, push, pull, reach, talk, hear, taste and smell.
- Must be able to lift, push, pull and move up to 50 pounds.
- Risk of exposure to blood borne pathogens and hazardous materials.
OTHER SPECIFICATIONS:
- Hourly (Non-exempt)
- Not eligible for shift or weekend differential
- 30 day notice is required for voluntary termination of employment
