Key Responsibilities
- Coordinate patient care activities to ensure timely and efficient discharge planning.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Assess patient needs and facilitate appropriate resources and referrals.
- Monitor patient progress and adjust care plans as necessary.
- Provide patient and family education on healthcare needs and discharge instructions.
- Ensure compliance with all relevant regulations, policies, and procedures.
- Education:
- Graduate of an accredited school of professional nursing (required); Bachelor of Nursing (preferred), OR
- Graduate of an accredited Master of Social Work program.
- Licensure:
- Current and valid license to practice as a Registered Nurse (RN) in the state of Texas OR
- Current and valid license as a Master Social Worker (LMSW) in the state of Texas (LCSW preferred).
- Certification:
- Certification in Case Management required within two (2) years of hire into the Case Manager position.
- Experience:
- Must have recent acute care case manager hospital experience.
- Three (3) years of nursing or social work experience in an acute hospital-based setting (preferred), OR
- Three (3) years of experience in a comparable clinical setting.
- Strong assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work effectively in a multidisciplinary team environment.
- Strong organizational and time management skills.
- Proficiency in healthcare technology and electronic medical records.
- This is a weekend position, requiring flexibility and availability to work during weekends.
