VNA of Ohio has an opening for a Pediatric Clinical Liaison/RN representing VNA at MetroHealth. This position is full-time.

A clinical liaison fosters relationships between patients and the facilities providing their care. With emphasis on pediatric care, the clinical liaison works with physicians and other health professionals to complete evaluations and consultations including obtaining necessary information to complete plan of care and referral coordination.

Responsibilities

Essential Functions:

Illustrative duties may include, but are not restricted to, one or more of the following:

  • Utilizes principles of the nursing process to assist in data collection, planning and evaluation of nursing care.
  • Demonstrates nursing knowledge and skills in the implementation of prevention, therapeutic and rehabilitative nursing care.
  • Utilizes effective interviewing techniques to define role, set limits and collect relevant data, history and assessment of the patient/family.
  • Identifies needs and assigns priorities in conjunction with the family to develop a comprehensive service plan.
  • Assists family in decision-making and goal-setting for the patient.
  • Makes periodic visits based on need.
  • Utilizes a follow-up system to evaluate the effectiveness of intervention strategies.
  • Initiates referrals and collaborates effectively with health care providers, third parties, patients and families based on specific role.
  • Explains processes, procedures and treatments to patient to gain cooperation, understanding, and ease apprehension.
  • Maintains awareness of comfort and safety needs of patient.        
  • Observes patient, records significant conditions and reactions, and notifies supervisor or Physician of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Responds to life saving situations based upon nursing standards, policies, procedures, and protocol.
  • Uses oral and written communication to convey pertinent information to members of the health care team in a timely manner.
  • Complies with company policies and procedures.
  • Completes all required forms, reports, time cards, itineraries, etc., accurately and within designated timeframe.
  • Recognizes and follows established lines of communication.
  • Performs other related duties based on departmental need
  • Receives orders, coordinates home care and completes patient assessments/ communication, and obtains all needed forms to complete a referral
  • Works closely with personnel in skilled nursing facilities, physician’s offices, home care coordinators and discharge planners in the community and with the VNA Intake staff, to coordinate a smooth, accurate, and complete submission of referral information
  • Works with sales representatives in the field to nurture relationships, build the VNA presence with existing and new referral sources, and schedule and manage visits in collaboration with the Sales Account Liaison
  • Ability to effectively present features and benefits of all VNA service lines
  • Communication with accounts through consistent feedback and follow up regarding patient status using available clinical data and key referral source data/reports

Qualifications

Qualifications and Requirements:

  • Completion of an accredited RN education program with a diploma, Associate’s or Bachelor's degree. Bachelor's degree in Nursing is preferred.
  • One year of experience in pediatric nursing, preferably with children with chronic illnesses or in the neonatal intensive care unit (NICU)
  • Knowledge of pediatric community resources
  • Experience or knowledge of community health preferred
  • One year of clinical experience in community and home health care
  • Knowledge of medical insurance, regulations, and Conditions of Participation (CoP) for certified home health care

Required Licenses, Registrations and/or Certifications:

  • Registered Nurse, licensed in the State of Ohio
  • CPR certification is required
  • Valid driver license
