We are currently looking for a Licensed Practical Nurse (LPN) to join our team. This position would be responsible for providing quality care and home health nursing care services under the direction of a Registered Nurse or an Advanced Practice Nurse.

Responsibilities

  • Supports organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers
  • Evaluates the needs of patients with life limiting illnesses and their families and provides nursing care services as directed by the Registered Nurse and as described in the interdisciplinary plan of care
  • Assists client to follow preventative and rehabilitative nursing procedures
  • Monitors quality and appropriateness of services provided
  • Establishes and maintains professional working relationship with health care providers and other members of the disciplinary teams in providing patient and family care
  • Adheres to agency infection control and safety policies
  • Adheres to the organization’s policy in regards to absenteeism and appearance
  • Omission of specific duties does not exclude them from this position if the work is similar, related or a logical assignment for this position
  • Qualifications

  • Graduate of an NLN accredited LPN program with current FL LPN licensure
  • 1 years of med/surg and acute care nursing experience
  • 1-2 years of homecare and or hospice experience preferred
  • Excellent verbal and written communication skills
  • Strong computer skills
  • Excellent organizational and interpersonal skills
  • Car and valid FL driver’s license
  • Willingness and ability to travel as needed
  • CPR Required
  •  Working Conditions/Physical Demand: Working indoors and outdoors with clinical equipment and use of phone and computer; Job characteristics: Client and Customer contact
