Under the supervision of the Director of Home Health & Hospice and in collaboration with other team members, delivers care to patients and families with needs ranging from basic to complex utilizing the nursing process. Creates a therapeutic and caring environment for patient and families. Practices in a manner consistent with the Franklin VNA & Hospice Practice model; the ANA’s Social Policy Statement, Scope and Standards of Practice, and the Nursing Code of Ethics; and relevant specialty standards of practice.


ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Interview the client and or family to obtain a comprehensive history and to obtain data about client/patient's developmental status and behavior.
  • Examine the client to obtain objective physical data.
  • Review available clinical data from referral and lab results, etc. as part of database.
  • Establishes and documents patient treatment regimen under supervision of RN or MD and implements treatment.
  • Reassess client status at established time frames and as indicated by the client status. Reports status and changes in status as appropriate.
  • Assess potential for development of crisis situations based on combination of subjective and objective cues.
  • Assists in developing and updating an individualized approach to deliver nursing care to the individual client and family.
  • Collaborate with interdisciplinary team to develop an integrated plan of care.
  • Acknowledge and implement the medical diagnostic and therapeutic orders.
  • Participates in patient treatment regimen, including administering medications (under the supervision of a physician and registered nurse).
  • Performs routine immunizations.
  • Performs phlebotomy and wound care under supervision of RN or Director.
  • Promote client dignity during provision of care.
  • Provide calm, supportive and therapeutic environment for clients and their families.
  • Assess the client response to and understanding of care provided.
  • Provide education to staff and facilities regarding client care.
  • Perform other duties assigned.

Requirements

EDUCATION AND EXPERIENCE

  • Graduate of an accredited school of nursing with a degree in licensed practical nursing.
  • Current Certification/Licensure as a Licensed Practical Nurse in the State of New Hampshire is required.
  • Minimum of 1 year of progressive relevant professional experience, preferably in a homecare setting.

SKILLS AND ABILITIES

  • Ability to deliver client care in a manner that is appropriate to patient’s age, physical ability and intellectual development. Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the client population served.
  • Ability to express or exchange ideas by means of the spoken word. Primary functions include activities in which incumbent must convey detailed or important spoken instructions to patients, physicians, families and other employees accurately, loudly or quickly
  • Ability to receive detailed information through oral communication and to make discriminations in sound.
  • Ability to plan and perform diversified duties requiring knowledge of nursing of the patient including age related changes, and the use of a wide range of procedures. Involves the exercise of judgment in the analysis of facts and conditions regarding individual’s problems, concerns or cultural factors to determine the plan of care and what actions should be taken, within the limits of standard practice.
  • Ability to perform job alone following standard practices and any of several procedures but refer questionable cases when they arise out of standard practice.
  • Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
  • Ability to work as a team and maintain positive working relationships with employees and healthcare professionals.
  • Ability to document patient care accurately according to nursing standards and plan of care.
  • Working knowledge of Microsoft Office software and other industry specific programs.
  • Must demonstrate ethical conduct and build trust with patients and family members.
  • Ability to follow safety guidelines as instructed by the agency.
  • Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc.
  • Ability to travel required. Travel throughout central New Hampshire is required.
  • Current drivers’ license is required.
  • Proof of automobile insurance is required.
  • Ability to pass a motor vehicle background screening is required.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • Job requires physical activities including but not limited to; standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, crawling, fingering, grasping, feeling, talking and hearing.
  • Job requires moderate physical work including the ability to exert up to 50 lbs. of force occasionally, 30 lbs. regularly.
  • Job requires close visual acuity including the ability to see at or within arm’s reach.
  • Job is subject to inside environmental conditions; low noise level.
