POSITION OBJECTIVE:

• Coordinates and supervises home health and hospice services to individuals and families in their assigned team within Stillwater's Multi-County service area.

QUALIFICATIONS:

• Must have current RN license to practice in the state of Indiana.
• Prefer a bachelor’s degree in Nursing or be working toward comparable degree.
• Must have at least two (2) years’ recent clinical experience, preferably with experience in home care.
• Must be comfortable working with home health and hospice patients with the emphasis on palliative care.
• Currently certified in CPR, must obtain within 6 months of hire if not certified. Currently certified in TB, must obtain within 6 months of hire if not certified.
• Must have interpersonal skills to effectively supervise staff and interact with patients and family members; must exhibit good public relation skills for interaction with physicians and other members of health care community.
• Must have ability to plan and implement changes and flex between roles, as necessary.
• Must have current motor vehicle operator’s license and have access to automobile with current automobile insurance coverage.
• Must be able to drive independently to all patient/family clients’ homes.
• Must be able to safely perform the essential functions of the job without posing a direct threat to the health and/or safety of self or others in the workplace.
• Must accept responsibility for a rotating 24-hour on-call.
• Must be willing to make scheduled visits on weekends, evenings, or as staffing needs arise.
