POSITION OBJECTIVE:
This position cares for patients in their homes in the MARION area
To provide safe personal care and support for the terminally ill, in the Patient's Home, under the direction of the Patient Care Director and RN Case Manager.
QUALIFICATIONS:
- High school diploma or GED required.
- Completion of 105 hours CNA Certification program (need a copy of Certificate).
- Current/active CNA License
AND
- Minimum six (6) months (one [1] year preferred) CNA/HHA experience, within the last twenty-four (24) months, in the home health, long-term care, or hospital setting.
OR
- RN nursing students who have completed courses with a grade of “C” or better and currently possess an active CNA License will be said to have met the above qualifications.
- Must be able to safely perform the job's essential functions without posing a direct threat to their health and safety or others in the workplace.
- Must have a current motor vehicle operator’s license and have access to an automobile with current automobile insurance coverage.
- For CNA, Home Care must be able to drive independently to all patients/family/ clients’ homes.
ESSENTIAL FUNCTIONS:
- Gives client care per agency policies and procedures, following a written Plan of Care.
- Appropriate and safe techniques in personal hygiene and grooming that include: bed bath, sponge bath, tub bath, or shower bath. Shampoo: sink, tub, or bed. Nail and skin care. Oral hygiene. Toileting and elimination needs; emptying commode and catheter bag; cleansing, removing, and replacing ostomy bag. Normal range of motion and positioning, ambulating with the use of a gait belt. Changing linens, adequate nutrition, fluid intake, and output.
- Assists patients with various rehabilitation activities under the direction of a
- licensed therapist or RN.
- Takes and records temperature, pulse, respirations, and blood pressure when assigned.
- Meal preparations.
- Assists patients with feeding and feeds patients unable to feed themselves.
- Observes patient’s condition and reports significant changes in status to the appropriate nurse.
- Computer documents care per agency policies and procedures.
- Manages care of assigned patients in a timely fashion.
- Maintains a clean, safe, and healthy environment in the patient care area.
- As a member of the interdisciplinary team participates in patient care conference
- as assigned to promote holistic care.
- CNA/Home Health Aide will maintain good personal hygiene and conduct themselves politely and professionally.
- CNA/Home Health Aides must maintain a therapeutic relationship with patients. This includes confidentiality of personal matters and patients’, coworkers’, and Agency business. Maintains respect for the patient, their privacy, and their property.
- Attends educational opportunities and in-services in accordance with agency guidelines to improve clinical practice.
- Must be willing to be cross-trained in other departments within the agency.
- Any other duties assigned by the supervisor.
- Adheres to all Stillwater Hospice policies.
- Adheres to Stillwater Hospice’s dress code policy.
- Adheres to Stillwater Hospice privacy practices as described in the Notice of Privacy Practices.
- Participates in quality improvement activities and other agency committees or task forces when requested.
