We are an independent, not-for-profit visiting nurse agency in RI and we’re growing! We’re looking to add more positive, hard working, reliable, and experienced professionals to our team! We are searching for a Per Diem Clinical Social Worker MSW to service patients from our Providence, North Kingstown, or Portsmouth, RI home office.
The Masters Clinical Social Worker is responsible for the initial and continuing assessment of patient/family psychosocial needs, and implementation of interventions in accordance with the interdisciplinary team plan of care to assist the patient/family in dealing with the psychosocial aspects of illness. This position reports to the Social Work Manager.
We are seeking a Social Worker to act as an advocate for patients' needs, providing support and working closely with the family to ensure their health care needs are satisfied. The Social Worker is primarily responsible for providing individualized care to our patients in their permanent homes, addressing their specific health care needs. A successful candidate will have the ability to think outside the box in order to create customized plans that will meet our clients' needs, be empathetic yet firm in their approach, and work well with both patients and caregivers.

Job Specifications:
- Per Diem / Per Visit Pay / Non-Exempt
- Monday-Friday Schedule
- Hours between: 8:30am-5pm
- Home office can be based from either Portsmouth, Providence, or North Kingstown, RI
- Currently servicing primarily home care & hospice & palliative patients in the EAST & WEST service area territories including the following areas of RI: Jamestown, Prudence Island, Newport, Middletown, Portsmouth, Tiverton, Little Compton, Bristol, Warren, Barrington, and Riverside, RI. May occasionally see patients in other parts of RI currently including: East Providence, Pawtucket, North Providence, Providence, Johnson, Cranston, West Warwick, Warwick, Coventry (not including Greene), East Greenwich, North Kingstown, and Saunderstown, RI
- Also, currently servicing some home care and hospice & palliative care patients in southeastern MA including: Swansea, Somerset, Fall River, Adamsville, & Westport, MA. May occasionally see patients in other parts of southeastern MA currently including Seekonk and Rehoboth, MA
The Ideal Candidate:
- Knowledge of and ability to apply social work principles and practices while exercising independent professional judgment within the realm of medical social worker.
- Experience with hospice & palliative care.
- Ability to contribute to the understanding and assessment of psychosocial factors in patient/family care.
- Knowledge of community resources and ability to access them as needed by the patient/family.
- Ability to counsel the patient/family and provide emotional support related to illness and recovery.
- Ability to assess the coping skills of the patient/family as they effect the patient/family's ability to participate in the interdisciplinary care plan.
- Ability to participate effectively with other members of the team in developing the interdisciplinary plan of care.
- Ability to support and assist coworkers in dealing with difficult patient/family situations.
- Works to ensure that clients are properly placed in care facilities.
- Prepares cases for court proceedings.
- Maintains lists for managed care reimbursement.
- Prepares reports on the care of the elderly.
- Reviews and updates social workers' databases.
- Resolves conflicts among family members.
- Helps patients find resources in their community.
- Ensures that the patient follow through with the plan of care.
Education and Experience:
- Masters level graduate of a school of social work accredited by the Council on Social Work Education.
- Currently licensed as a Clinical Social Worker in the states of Rhode Island and Massachusetts. If not licensed in Massachusetts at time of hire, employee has until their 1 year anniversary to obtain reciprocal Massachusetts licensing.
- Minimum one year experience in medical social work preferably in a home care setting.
