About the job:

The Nurse Practitioner provides primary care to participants. Performs physical assessments, semi-annual reassessments of participants and develops and implements appropriate plans of care for program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT) under the direction of the Medical Director. Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Volunteers of America National Services standards.

Pay: $43.27 to $50.00/hr - Depending on previous experience

Schedule: PRN, occasional weekends

  • Education: Must be licensed where applicable by the State.
  • Graduate of a school of professional nursing required. Licensed as Registered Nurse (R.N.) in the State. Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing. Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred.
  • Certification: Must have current Cardiopulmonary Resuscitation (CPR) certification.
  • Must have a valid driver’s license, proof of insurance and have means of transportation.

Preferred Skills:

  • Experience working on a multi-disciplinary team in a hospital, nursing home or community-based setting is preferable. Two years’ experience as a nurse practitioner in a geriatric setting desirable. Shall have either training or related experience in the job assigned.

Essentials: 

  • Performs in person comprehensive history and physical on new participants. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
  • Acts as and engaged member of the Interdisciplinary Team (IDT). Regularly attends meetings and provides meaningful input.
  • Reassesses participants on a semi-annual and as-needed basis and coordinates with the IDT to update the plan of care for each participant.
  • Maintains participants’ medical record and fulfills Senior Community Care charting and reporting requirements.
  • Assists with the development of, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.
  • Is responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.
  • Manages and reviews participant’s records, charts, pertinent information and staff’s documentation. Documents as needed in medical records according to Senior Community Care standards.

The Volunteers of America National Services Colorado Float Team is a team of experienced professionals that assist in the facilities that we serve, such as the Senior Community Care Program for All-Inclusive Care for the Elderly (PACE), Horizons Health Care Center, Valley Manor Care Center, the Homestead at Montrose, and Home Health of Western Colorado. As a PRN employee, you will enjoy ultimate flexibility and work as little as 4 shifts per month or full-time hours, it is YOUR choice. One (1) year of experience is required - you must be willing to work at any VOA facility that has a need. Love to travel?  Interim assignments to facilities in other states may be available!  To travel, you must have a multi-state nursing license and/or the ability to obtain a state license in the state of the assignment.
