Job Opportunity: Social Worker in Delta & Montrose Counties
Are you a compassionate social worker looking for a meaningful role in a beautiful rural community? We are seeking a dedicated Social Worker to support patients and their families across Delta and Montrose Counties. This position offers a unique opportunity to work 20 hours per week in each location, with a flexible schedule and the freedom of a "mobile office." You'll spend your time on the road, visiting patients and their families in their personal living environments.
As part of an interdisciplinary care team that includes social workers, chaplains, nurses, physicians, and certified nursing assistants, you will play a crucial role in improving the everyday lives of those who need help the most. Your advocacy will involve providing education and resources to patients and their families, ensuring that their care honors their goals and wishes.
Key Responsibilities:
Work with an interdisciplinary team to deliver high-quality care to patients in the community.
Provide education, resources, and support to patients and their families in various settings, including homes, nursing homes, and assisted living residences.
Advocate for patients' needs and ensure their care aligns with their personal goals.
Qualifications:
Minimum of one year of experience in healthcare social work.
Experience working with the elderly (paid or volunteer) is preferred.
Excellent interpersonal and communication skills.
Ability to live in and commute between Delta and Montrose, CO, enjoying drive time.
BSW required; MSW with state licensure preferred.
Must participate in on-call, holiday, weekend, and after-hours rotation.
Compensation and Benefits:
Hourly pay ranging from $27.00 to $31.00, with shift differentials as applicable.
Premium pay for Licensed Clinical Social Workers and/or bilingual fluency.
Mileage reimbursement at 60 cents per mile for work-related driving.
