Position Title: Shared Living Coordinator
Program: Adult Services
Reports To: Director Special Projects
Hours/Schedule: Varies core hours 8a-4p M-F
Hourly/Salary Status: Exempt
Position Overview:
The Shared Living Coordinator is responsible for assisting with the development, and oversight of multiple Shared Living Providers. This includes providing ongoing support to the Shared Living Providers. This role requires a high level of communication with the Shared Living Manager and is integral to the success of the Shared Living Program.
Duties and Responsibilities:
In alignment with the Agency’s Four Cornerstones of Member Care, Driven Program Integrity, Manage and Develop Staff and Facility Management, this position is accountable for the following key duties and responsibilities:
Member Care:
Participate in PCP meetings, Shared Living Provider meetings/trainings, communicate concerns and report outcomes back to the Assistant Director of Residential Services in a timely manner.-
Participate in planning meetings to ensure appropriate goal development and implementation for members residing in Shared Living homes.

Communicate and interact effectively and professionally with members, providers, family members, guardians, case workers, and other stakeholders.

Ensure best practice and quality standards in service delivery.

Ensure the provision of coordination and advocacy for access to services, supports, and resources.

Provide consultation and support to Shared Living Providers as needed.
Program Integrity:
Complete all necessary documentation and reports to ensure timely submission of all documents to stakeholders according to the standards of the agency.-
Adhere to a regular schedule of bimonthly home visits

Maintain accurate documentation of home visits, and other related contact with the Shared Living providers

Maintain all records in accordance with the agency standards and the Office of Aging and Disability

Ensure all OADS and MaineCare requirements are adhered to as related to Shared Living.

Ensure compliance with all Waban policies and procedures.

Fully understand and abide by the agency’s code of ethics
Staff Development:
Aids Shared Living Providers with the appropriate tools and support to effectively manage the responsibilities of their independent program.-
Leads the development of new Shared Living Option.

Supports Shared Living Providers in all areas related to their responsibilities

Assure that Providers attend ongoing professional trainings and provide clinical consultation as needed.
Facility Management:
Adheres to all Waban safety protocols.-
Ensures the Provider(s) home reflects the Maine DHHS standards and Waban’s standard for a safe living environment. Shares any concerns with the living space with the Director of Special Projects in a timely manner.
Additional Duties:
Position Competencies (Knowledge/Skills/Abilities):
Experience and knowledge of the Shared Living Option preferred.-
Experience working with individuals with a diagnosis of Autism, Intellectual Disabilities and mental health challenges.

Problem solving skills and strategic planning highly desirable.

Must be able to problem solve and offer solutions to challenging situations.
Additional Qualifications:
Ability to speak, read and write in English.-
B.A. Degree preferred. Degree in Social Services or a closely related field preferred.

Valid driver’s license required.

Must have a reliable vehicle with proper insurance, current registration, and a valid driver’s license from state of residence.

Some early evening hours may be required to meet with Providers and members

Excellent interpersonal and time management skills. Must have professional level writing skills to maintain documentation standards, reports, and any other related responsibilities.

Effective computer skills including navigating multiple systems and keyboarding
Physical Requirements:
While performing this job the employee is regularly required to sit for extended periods of time.-
Ability to travel.
