What are the responsibilities and job description for the Speech Language Pathologist position at Waga Health Group?
Identify, assess and provide treatment for communication and swallowing function and their disorders, complete required documentation and also participate in program planning and implementation, discharge planning. Identify potential residents in need of speech-language pathology services through screening protocols. Evaluation of deficits, strengths and needs required for communication and swallowing. Develops intervention plans; sets goals and determines resident’s progress potential based on resident’s ability to reach full potential. Adapts activities, environment, tools, materials according to the needs of the resident. Modifies resident’s response to intervention and modifies treatment plan as needed. Provides counseling to the resident, family, caregivers and staff regarding communication/swallowing status, needs and emotional adjustment to deficits. Screens auditory function and makes appropriate referrals. Formulates discharge plan and recommendations when services are discontinued.
