Oversee the Options for Long Term Care Case Management Team and coordinate all the functions of the Long-Term Care Waiver and State programs according to contract and program regulations. Responsible for the direct supervision and development of Options for Long Term Care staff and ensures that performance standards are met as identified by County, State or Federal Government guidelines.

$78,406.21 - $99,292.28 annually plus benefits (see benefits tab on the job posting). The position will normally start at $78,406.21 annually
This position is exempt from the minimum wage and overtime requirements of the Fair Labor Standards Act.

This range encompasses the nine pay steps available should you receive the position and demonstrate satisfactory performance over time.

Supervision (25%):
• Directly supervises the Options for Long Term Care unit staff.
• Provides leadership, guidance and support to staff ensuring a high level of performance and compliance with County, State and Federal rules and regulations.
• Addresses employee concerns and resolve problems in collaboration with Department Personnel and County Human Resources.
• Assists in developing educational and professional development opportunities for staff.
Case Management Oversight (25%):
• Monitors and evaluates workflow using knowledge of business functions, program requirements and economic conditions to establish unit goals and timetables.
• Assigns and schedules work within the unit using assigned employee’s performance and level of skill to assure coverage and efficiency in caseload handling while accomplishing program goals accurately and timely.
• Uses tracking data to identify trends in customer volume and the need for making changes in work assignments to adjust accordingly.
• Conducts individual and group conferences to set and monitor deadlines.
• Manages all the components of the Home and Community Based Services Waiver Programs, the Nursing Facility placement process, the State only long-term care programs and the Private Case Management functions of the OLTC.
• Communicate effectively and work in conjunction with local, state and federal entities to ensure client access to services.
Compliance (20%):
• Establishes and enforces standards for quality and quantity of work performed by workers in the OLTC unit using professional standards set primarily through state rules, statutes, and manual regulation.
• Develops systematic methods to monitor work to ensure compliance.
• Maintains quality assurance levels for all Options for Long Term Care Programs through case audits, client satisfaction surveys and case consultations to ensure all program and contract requirements are met.
Budget (10%):
• Participates in the budgeting process, demonstrating a clear understanding of the funding mechanisms of the Options for Long Term Care program.
• Maintains an accurate client reporting system and submit timely reports as required by contract and regulations and internal monitoring requirements.
Outreach (15%):
• Serves as an advocate for clients, families, and/or providers during conflict situations and assist with problem solving the best solutions to meet clients’ long-term care needs as well as for meeting other needs that may arise.
• Coordinates the Options for Long Term Care Advisory Committee and conducts regular meetings as required.
Other Duties as assigned (5%):
• Follow policies and procedures, the Weld County Code of Conduct and adhere to strict confidentiality guidelines and appropriate release of information outlined in both.

• Bachelor’s degree in a Human Services or related field, such as Gerontology, Psychology, Rehabilitation, or Sociology.
• Six (6) years full time professional work experience three (3) years of which must include professional program management experience in a public or private human services organization.
• Experience in the field of aging OR rehabilitation OR long-term care.
• Demonstrate case management experience.
• Two (2) years’ experience supervising and managing employees that includes directing work activities and evaluating employee performance.
• Candidate must have a valid Colorado Driver's License and Liability Insurance.
• Candidate must provide own transportation and be willing to travel throughout Weld County.
• Candidate must pass Human Services Background Checks.
• Candidate must possess the ability to enter data into several different computer programs.
• Candidate must possess the ability to learn and apply new computer skills on a regular basis.
• Candidate must possess the knowledge of computer programs such as Microsoft Windows and Word, Excel, and Outlook.
• Candidate must possess the ability to read and comprehend instructions, correspondence and department policies and procedures.
• Candidate must possess the ability to write correspondence.
• Candidate must possess the ability to effectively communicate in one-on-one and small group situations to customers, clients, and other employees of the organization.
• Candidate must possess the ability to apply math concepts and calculations in the work environment.

WORK ENVIRONMENT

• Work may involve time away from a standard office environment.
• Work may occur in various community settings that are potentially hazardous and may require additional precautions, such as personal protective equipment and/or requesting assistance from emergency medical personnel or law enforcement.
• After hours and on call work situations may occur on occasion to provide support to families that need services outside of normal business hours.
• This position is in-person with a possibility for limited telework opportunities when needed.
