Job Summary:
The Ambulatory Nursing Informatics Specialist, under the supervision of the Director, performs services. Is responsible for overseeing the design, implementation and support of ambulatory clinical applications. Serves as a primary resource for both providers and ambulatory staffs’ informatics needs and provides ongoing support for their clinical systems. Works collaboratively with IT leadership to monitor issues and mitigate risks. Leverages leadership skills and clinical knowledge to help ensure the electronic health record processes are aligned in providing the highest quality of safe, efficient, evidence-based, patent and family centered care that is.
Nursing Informatics:
- Serves as liaison between the Ambulatory practices, Inpatient clinical teams and IT to provide effective communication across all service lines
- Strategizes workflow discussions for redesign and implementation of rollout efforts.
- Complies with standards for change control and work plans. Ensures appropriate communication and documentation is complete in order to support application quality and development
- Manages and participates in clinical standardization efforts for the purpose of building clinical informatics systems, workflow analysis, and evidence-based content in accordance with organizational policies and procedures.
- Leverages internal and external resources and tools to deliver a high-quality electronic health record. Coordinates problem identification and resolution
- Supports end user adoption and sustainment of the HER.
- Focuses on clinical utilization and workflow including patient safety and clinical outcomes through safe EHR use and practices.
- Works directly with clinicians and clinical leadership to proactively assess adoption and sustainability of the electronic record.
- Collaborates with multidisciplinary team members to assess, inform, educate, and continually improve effectiveness and productivity of clinical and business programs.
- Anticipates and identifies complex clinical and business processes and ambulatory issues, monitors and measures the risk factors. Communicates risk factors to informatics leadership and stakeholders. Manages projects to ensure the implementation of continuous quality control of process and deliverables.
- Works with clinical staff to implement streamlined and efficient workflow changes (documentation, EMR tools, etc.).
- Participates in planning and provides support for population health programs, including but not limited to, patient-centered medical home, transitions of care, health maintenance/prevention, and chronic disease programs, including those focused on diabetes, COPD, hypertension, etc.
- Conducts data analysis to identify trends and patterns related to care outcomes. Develops action plans to address negative findings.
- Conducts assessments of current population health/disease management program needs and/or recommends opportunities for implementation or enhancement of such.
- Identifies barriers to change and accepts and manages ambiguity/complexity.
- Is visibly involved in obtaining feedback, incorporating staff requirements into service design and advocating for technical solutions to clinical and business processes.
- Provides support and coaches stakeholders in effective process analysis and improvement techniques and in using data to assess and improve unit performance.
- Participates in multiple aspects of electronic health record development and implementation. Participates in training, go-live and elbow support, reporting, business and clinical needs prioritization for the electronic health record.
Hours: 40 hours per week, Day Shift
Qualifications:
- MA licensed RN with BSN or equivalent preferred.
- 1-3 yrs. Previous Nursing Informatics experience required; 3-5 yrs. Epic experience preferred.
- Competency in Microsoft Word, Excel and Outlook.
- Strong written and verbal communication skills.
