POSITION SUMMARY: Care Coordinators are responsible for working collaboratively with their clientele, all their support systems to include community providers to ensure support for overall health and wellness.
ESSENTIAL ACCOUNTABILITIES:
Works in a team environment to meet deadlines and needs of consumers.
Carries an individual caseload of approximately 45 consumers.
Note: Specialized populations AOT carry a caseload of a maximum of 15 consumers and the DOH Specialized Population carries a maximum caseload of 25.
Discusses the program and services offered to the participant, from the point of intake to discharge.
Assures timely intakes, assessments, and referrals.
Is directly responsible for accessing care coordination services for the participant.
Coordinates all assessments from the various disciplines, developing case plans, and referrals for treatment, including, but not limited to, health, mental health, substance abuse, social, vocational educational housing, and employment services.
Ensures that all participants receive quality program services and referrals to other community services within the policies and guidelines of the Care Coordination Program.
Maintains all case records in direct accordance with the Care Coordination Program standards of practice.
Obtains all necessary records from hospitals, doctors, and other agencies, both past and present that were/are involved with the participant.
Follows up on all assessments recommendations.
Is responsible for meeting with the participant on an as-needed basis for follow up and counseling.
Conduct field-based community visits.
Prepare and revise care plans to reflect client needs and personal goals with a focus on maintaining health and wellness.
Complete all required monthly documentation as required to ensure appropriate and accurate billing.
OTHER RESPONSIBILITIES:
Maintain confidentiality at all times.
Participate in the activities of other staff members in their absence or during periods of staff shortage.
Represent the agency at meetings, the training not otherwise specified.
Ability to work a flexible schedule as work schedule and locations are subject to change.
QUALIFICATIONS: Bachelor’s degree and two years’ experience in Human Services or related field required; New York State driver’s license and access to a vehicle required.
