We are seeking a passionate Case Manager to join our team and play a vital role in supporting our clients on their journey to improved health and well-being.
Responsibilities:
- Client Onboarding: You'll be the first point of contact for new clients, walking them through the sign-up process and ensuring a smooth transition into our services.
- Comprehensive Assessment: Conduct thorough assessments to understand each client's unique needs and challenges.
- Personalized Service Plans: Develop individualized service plans in collaboration with the client, service coordinator, and other relevant parties.
- Collaborative Care: Foster strong relationships with service coordinators, clients, families, lead caregivers, and other healthcare professionals to ensure seamless care delivery.
- Monthly Reporting: Participate in monthly meetings and provide comprehensive updates on client progress.
- Communication & Advocacy: Keep the office director informed of new client authorizations and new caregivers assigned to your caseload. Advocate for your clients' needs and ensure they receive the best possible care.
- Detailed Documentation: Enter accurate and detailed notes after every client interaction to maintain clear and comprehensive client records.
- Client Support: Be a source of support for your clients, assisting them with any needs or questions they may have.
Qualifications:
- Associates degree or Bachelor's degree in Social Work, Human Services, or a related field.
- Must have a valid drivers license and reliable transportation
- Minimum 1 year of experience in case management or a similar role (internships included).
- Strong assessment and planning skills.
- Excellent communication, interpersonal, and collaboration skills.
- Initiative-taking, independent worker with a goal-oriented mindset.
- Proficient in documentation and record keeping.
- Compassionate and empathetic with a dedication to client well-being.
