How you make a difference
The Discharge Planner is responsible for coordinating pre-release or post-release case management and assessing the discharge planning needs of all inmates. Working under the direction of the Health Services Administrator or Program Manager, they will implement re-entry care plans and provide ongoing support to individuals served in accessing community behavioral health resources, substance abuse treatment, housing, employment, education assistance, and other relevant needs. The Discharge Planner will also coordinate community services to ensure successful re-entry into society.
Key Responsibilities
- Provides discharge planning and case management services for individuals on the behavioral health caseload, including pre- and post-release interviews and coordination of community resources.
- Ensures continuity of care by coordinating medical benefits, social services, and outside behavioral health/substance abuse services, as well as obtaining medication scripts and coordinating housing placement.
- Conducts groups and educational sessions on life, job, and parenting skills, and may assist in the job shadow portion of the interview process and orientation/training of new employees.
- Documents relevant clinical information and data for quality initiative purposes to improve patient care.
- Collaborates with agency resources and community providers to provide ongoing support to all individuals served in the areas of behavioral health resources, substance abuse treatment, employment, social relationships, education assistance, and other relevant needs.
Qualifications & Requirements
Education
- Completion of an accredited social work, RN or LVN program
Experience
- One year experience as a Discharge Planner in an acute setting or case management/care coordination in a mental health facility
- Previous correctional/detention facility experience
Licenses/Certifications
- Current (and must maintain) lifesaving BLS certification through AHA
- Unrestricted current license in state of practice
