Bereavement Coordinator

We are currently hiring a full-time Bereavement Coordinator for WesleyLife Hospice. Our Hospice team supports patients and loved ones with our patient-centered approach. We extend personalized patient- and family-centered care to encourage patients to live life on their terms.

As our Bereavement Coordinator your responsibilities will include:

• Provides education to patients, families, and others about services available to the bereaved.
• Works with the interdisciplinary group (IDG) to ensure initial bereavement comprehensive assessments are included in plans of care.
• Initiates bereavement services for family members and others upon the death of loved ones.
• Provides ongoing bereavement services for family members and others for up to one year following the death of loved ones.
• Communicates routinely with Social Workers and Spiritual Care Counselors regarding ongoing bereavement needs identified as end of life care progresses.

Experience

• Valid driver’s license, reliable transportation, valid insurance, and clean driving record
• Ability to pass a pre-hire background check, pre-employment health assessment, and drug screen

• Two years of full-time related work experience required, including one year of full-time work experience in bereavement.

Education

• Bachelors or better in Human Services or related field
