What are the responsibilities and job description for the LPN Visit Nurse, Home Health position at WesleyLife?
A day in the life of a LPN Visit Nurse for our Home Health team:
- Travels to complete visits for each assignment
- Meet daily productivity expectations set by the Home Health Clinical Manager based on patient acuity
- Complete documentation within expected timelines
- Coordinate care and communicate with the Home Health team
The pay range for this position is $23.25 - $29.07 / hour. Starting pay rate will be based on years of experience.

As a nurse with WesleyLife at Home, you’ll receive the onboarding and training you’ll need to quickly become a successful part of the team. Our case manager caseloads are around 35 clients, and our visit nurses and home care aides help manage those caseloads. You’ll work a full day, but the number of clients you’ll see in a given day will depend on the amount of travel necessary.
