JOB SUMMARY: The staff physician is a credentialed, licensed physician who is a salaried (exempt) employee who provides primary care medical services in a Patient Centered Health Home. HIV specialty a plus!

This role will be responsible for:
- Provide medical services to include acute care, periodic re-evaluations, and management of chronic conditions. May include services in specialized areas (such as obstetrics and HIV/AIDS) as required.
- Provide leadership within Care Team to encourage effective support for patient self-management, effective coordination of care across health system, reduction of unnecessary utilization of medical services, optimal health outcomes, and maximized experience of care received through West County Health Centers.
- Provide leadership in population management of the Care Team patient panel using population data.
- Participate in Care Team meetings to include daily huddles, monthly care team meetings, complex care case conferences and other related meetings.
- Participate in quality improvement, quality assurance, and innovation activities to ensure optimal level of care delivery at West County Health Centers.
- Actively work with Care Team to optimize efficiency, team collaboration, and communication.
- Assist in the coordination of warm hand-offs or "Transfer of Trust" between Care Team, Behavioral Health Staff, or other support services during the office visit.
- Document care in the electronic medical record and appropriately complete necessary documentation of care provided.
- Maintain hospital privileges, licensure, CPR, and DEA certification, as well as other certifications and licenses as required.
- Share on-call and hospital rounding responsibilities with other physicians, and provide billing information for all off-site encounters.
- Full-time physicians will generally work four on-site clinical days, plus on-call, and share Saturday clinic hours. Hours and schedules will be as assigned.
- Maintain ongoing clinical competence through formal and informal Continuing Medical Education activities.
Experience & Qualifications:
- MD or DO licensed in California, with Board Certification and appropriate experience
- Current CPR certification
Physical Requirements:
- Must be able to move freely about the office and carry light loads (up to 25 pounds)
- Prolonged standing and occasional sitting required.
- Ability to work with office equipment, including computers
