JOB TITLE: Case Manager
FLSA: Exempt
DEPARTMENT: Homeless Services
REPORTS TO: Service Coordinator
LOCATION: White Plains
SALARY: $38,000 TO $52,000 PER YEAR
SUMMARY: Provides a range of case management and supportive services to families. Reports to the Casework Supervisor.
DUTIES AND RESPONSIBILITIES:
Provide case management, including needs assessment, crisis intervention and appropriate counseling for families.- Orients new families to the program, including resident’ rights, rules and regulations, etc.
- Assist families in the development of a service plan, monitor the progress and advocate on behalf of the families, to secure all social service entitlements; refer families to identified services.
- Evaluate and document resident’s conditions, daily living skills and family interactions.
- Develop a plan to help families improve in areas of concern.
- Complete and maintain case records and required statistical data and reports.
- Refer resident issues to the Service Coordinator.
- Other duties as assigned.
EXPERIENCE, EDUCATION AND SPECIAL REQUIREMENTS:
BA degree in Social Work or 8 years’ casework experience or equivalent combination; knowledge and ability to access service delivery system; experience in counseling. Work with high risk and/or chemically dependent families preferred; CAC desirable, bi-lingual a plus.
