Family Nurse Practitioner (FNP) needed for outpatient community health center in Windham County, CT!
Job Function:
- 100% outpatient position, focused on seeing patients of the health center
- Slow ramp up during training, but eventually a full panel considered 15-20 schedule patients per day
- Patients of all different demographics and socioeconomic status - work collaboratively with other providers
- Evaluation, diagnosis, medication assessments and adjustment, patient education, and treatment planning
- Hours: Monday-Friday, 8-5
- FQHC/Community health - student loan forgiveness eligible
- One location has 40% Spanish population and prefer medical or fluency in Spanish
Job Requirements:
- CT State license, DEA, CSR (active or applied preferred) but not required
- Bilingual language skills in Spanish preferred, but not required
