As the Resident Care Manager (RCM) you will provide oversight and direction for the resident health care functions to ensure high quality resident care and a positive working environment for assigned neighborhoods, Terrace and / or Home Care services.
In this capacity, you'll play a pivotal role in assessing residents' needs, coordinating care plans, and fostering positive relationships with residents, families, and staff members.
Conducting thorough evaluations of residents' physical and mental health, family and community resources, and the physical environment is a key aspect of your responsibilities.
Collaborating with interdisciplinary teams, you'll develop, implement, review, and revise individualized care plans / service plans for each resident, ensuring assessments are conducted regularly and whenever there is a change in condition.
Overseeing resident medication administration, maintaining accurate medical records, and delegating nursing tasks appropriately are essential components of your role.
Additionally, you'll communicate with physicians to plan care for residents as needed and coordinate services with outside medical service providers.
Building strong relationships with residents, families, staff members, and supporting functional groups through proactive communication and feedback mechanisms is crucial to maintaining a high quality of care for residents.
QUALIFICATIONS
- Must be a Registered Nurse in the state of Oregon, BSN preferred.
- Minimum 2 years nursing experience, long term care setting preferred.
- 1 year supervisory experience.
- Able to interact well with staff as well as residents and their families.
- Ability to coordinate and manage a variety of details; good time management, organizational and project management skills.
