JOB DESCRIPTION
OBJECTIVE: Assists nursing personnel in provision of basic care for residents and necessary tasks and functions in compliance with Policy and Procedures
ORGANIZATION: The Certified Nurse Aide functions as a member of the health care team under the direction of the RN or LVN and reports to the Nursing Supervisor in conformity with Agency and regulatory policy.
QUALIFICATIONS:
- Candidate must have current and valid Texas Certification for Nursing Assistants.
- Compassionate, mature, sympathetic and professional at all times.
- Good organization and communication skills
- Ability to read, write, understand and carry out directions.
- Candidate will have the physical ability to perform job‐related duties, which may require lifting, standing, bending, transferring, stooping, stretching, walking, pushing and pulling.

RESPONSIBILITIES: Assists elders in the following areas:
- Keep call lights within reach at all time.
- Personal care functions including:
- Bathing (bed bath or shower)
- Toileting (bedpan, urinal, commode and/or toilet)
- Grooming (shampoo, nailcare & shaving)
- Oral Hygiene (including brushing, flossing, denture care)
- Assist with dressing and undressing
- Assists with feeding provide water/fluids throughout your shift.
- Measuring and recording intake and output.
- Weigh elders standing or in wheelchair.
- Assists in turning and positioning every 2 hours checking briefs every 2 hours and changing as needed.
- Proper transfer techniques including Hoyer lift transfers.
- Demonstrates appropriate knowledge for safe use of medical equipment (cane, crutches, walkers, Hoyer Lift, side rails, brace, splints).
- Helps keep elders’ rooms clean and supplied. Knock on doors before entering.
- Make and change beds (unoccupied and occupied).
- Transports elders, supplies and equipment as needed.
- Assists nurses and other personnel as needed.
- Ambulate elders who require minimal assistance.
- Perform skin checks and notify Nurse of any abnormalities in skin integrity.
- Answer elders calls and takes appropriate action.
- Measures and records vital signs/weights and reports variations in vital signs to Nurse.
- mmediately report any changes in elders’ condition or incidents to the Charge Nurse. Report suspicion of abuse or actual abuse immediately. Stop and Watch.
- Participates in care plans with other members of the healthcare team as appropriate
- Maintains confidentiality in relation to all elders, healthcare staff and documentation.
- Meets annual in-service requirements in accordance with Agency policy and state regulations.
- Maintains and enhances skill through attending appropriate staff development training
- Appearance is professional and complies with agency dress code.
- Maintains a cooperative manner towards elder/family and all members of the healthcare team.
