HealthMark Rehabilitation is an inhouse therapy company, serving the Skilled Nursing Facility patients and residents of HMG facilities.
We’re currently seeking a Part-time Occupational Therapist to evaluate and treat residents and short-term patients at our Willowbrook facility and split time with our sister facility Stallings Court in Nacogdoches, Texas.
Responsibilities include:
· Develop and implement the Occupational Therapy plan of care for facility residents and short-term patients.
· Provide supervision, as needed, for staff Occupational Therapy Assistants in delivery of the OT plans of care.
· Assist as needed with patient screenings for the need for Occupational Therapy.
· Communicate progress and any barriers toward OT goals with interdisciplinary team members.
· Complete timely OT documentation in compliance with regulatory, licensing, and payor requirements.
· Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
· Provide and document patient, family, and caregiver training regarding each OT patient’s specific plan of care and recommendations for follow up.
· Perform additional duties as required.
Qualifications include:
· OT degree from an accredited Occupational Therapy program.
· Current Texas license in good standing as an Occupational Therapist.
· Meet physical requirements to deliver OT responsibilities.
· Experience in a geriatric rehabilitation setting preferred.
· Successful completion of hiring process / background check.
