Overview
Scope of Position: The Human Services Professional- Homes (HSP) manages the case of each child in their assigned homes, serves as an initial point of contact for serious child-related issues and liaises with the state and juvenile court system, as well as with private placement custody holders. WinShape Homes HSPs serve alongside House Parents and the Clinical Care team to assist in ensuring best practices and state compliance with state licensure rules are being met.

Responsibilities
- Manages cases of children, documents all information related to the child, and develops a comprehensive plan for services that incorporate clinical interventions and ensures documentation of the child’s care is complete for all records. Assists House Parents and Clinical team in identifying appropriate interventions as needed for each child and is the liaison between all involved parties for the child in working towards the identified goal.
- Supports new children in the home by gathering information, and building relationships with referral source in order to strengthen family connections. Participates with team to determine appropriate placement for the child and completes initial service plan and assessments within specified time periods.
- Acts as a liaison between program and the juvenile court system and as needed will testify in court and participate in panel reviews, meet with custody holders, legal counsel and child advocates, and manages court calendar for each child and court documents.
- Serves as on call to respond to mental health crises or serious incidents and as needed travels to home to assess specific child’s needs and communicates with HSP supervisor as soon as practical for all incidents.
- Supports the children and House Parents and visits the home at a minimum of twice a month, one visit being to meet 1:1 with the children and serves as a resource for parents and helps problem solve and find outside resources as needed. Maintains comprehensive knowledge of WSH Operations Manual and ensures practices within the home align with program policies and best practice standards. If there are concerns that actions do not follow policies or standards they will be shared in team meetings. If actions continue, HSP will notify Supervisor who will report to the Director. The Director will then address the issues directly with House Parents.
- Responsible for ensuring up to date records are maintained for all assigned child files and works alongside Licensing and Training Specialist to complete assigned duties related to ensuring licensing requirements have been met for the year.
- Participates in CQI-related activities aimed at evaluating impact of service delivery (case reviews, data collection, data analysis, etc.)
- Conducts self in a professional manner with knowledge in multiple aspects of child welfare and obtains appropriate training and education to strengthen skillset. Assists team in the execution of staff trainings and retreats. Participates in HSP and Group Care meetings, WinShape Homes trainings, ONE Conference, Fall Rally, and other WinShape Foundation events.
- Acquires and manages training certification to lead other team members in trainings associated with Group Care operations and the care of children.
- Ensures the team of HSP, Counselor and House Parent meet at a minimum of once a month to review service plans, treatment goals and implementation and fidelity of Together Facing the Challenge strategies. HSP serves as a equal member of the team and owns and process or documentation needs from the meeting. HSP, or any other member of the team, can request Director assistance in determining final decision when no consensus in the team is able to be achieved.
- Represents and promotes WinShape Foundation and Homes at all times in day-to-day activities and exemplifies our core values.
- Performs other duties and tasks as assigned by the Supervisor, Human Service Professional - Homes and Director, WinShape Group Care - Homes.
Qualifications
Spiritual:
- A self-professing Christ follower, actively involved in a Christian faith community and actively pursuing Christ on a consistent basis.
- Exemplifies servant leadership as modeled by Jesus Christ
- Commitment to discipleship, prayer and Bible Study
Education:
- Bachelor’s degree in social work, psychology, education counseling or related field (Master’s degree in social work preferred)
Professional:
- 2-4 years work experience in a child welfare setting.
- Ability to manage multiple tasks, complex problems, and projects simultaneously.
- Maintains full time hours and must be present in assigned office when not conducting field work unless otherwise arranged with supervisor
- Willing and able to work nights and weekends
- Possess a valid driver’s license and has an acceptable driving record.
- Exemplary planning and time management skills
- Possesses the ability to use discretion and confidentiality
- Demonstrated outstanding interpersonal and communication skills including telephone, email, written documentation, and face-to-face communication
- Proficient in Microsoft Office software (Word, Excel, Outlook)
