This continuum is designed to meet the complex needs of each individual and creates a continuum of services that are able to meet the needs of children, adults, and families through a continuum of services utilizing an integrated care model. Title: Resources and Care Coordinator
Salary: Starting at $22 / hour depending upon education, and experience.
Responsibilities
- Work with youth and family to plan, implement and evaluate service provision and navigate treatment needs
- Provide active care management including linkages to community resources including for needs in areas of social determinants of health
- Ensure the coordination of support meetings and crisis plans for individuals served
- Coordinate development of cross-systems crisis plans, intake/assessments, intervention and outcomes and any other applicable documentation of services provided
- Participate in recurring meetings with leadership and ACT team
- Maintain linkages and relationships with community partners
Qualifications
- Bachelor’s degree in social work, counseling, psychology, or human services field with a minimum of two years’ experience providing services to people with mental health or co-occurring behavioral health challenges
Assertive Community Treatment Services for Youth (Youth ACT)
The program operates throughout Cape May and Atlantic County and delivers a multidisciplinary mobile team to provide intensive and immediate access to treatment services in the community. The program works with youth ages 5-20 who present with significant mental health needs, challenging behaviors, and/or are at risk of psychiatric hospitalization or out of home care.
