What are the responsibilities and job description for the Therapist LCSW position at XtreamAdminz?
As a Licensed Clinical Social Worker and one of the first members of our clinical team, your primary responsibility will be to assess the behavioral health of our patients. You will then develop and manage personalized care plans that cater to their specific behavioral health needs. Additionally, you will play a crucial role in coordinating their care across both Our Company and the Health System's care teams.
• Master's degree or higher in Psychology or a related field,
• Valid Licensed Clinical Social Worker (LCSW) certification/license in Illinois (preferably with a willingness to obtain it in multiple states).
• At least 3 years of experience in Digital Healthcare and be proficient in Cognitive Behavioral Therapy (CBT) and Dialectical Behavioral Therapy (DBT).
• Minimum of 3-5 years of experience in practicing CBT, experience in Cardiac Health, and expertise in Motivational Interviewing and Mindfulness are desired.
• Experience counseling adults and geriatric patients, managing chronic pain and hospice care, and working in integrated clinical teams focusing on heart and vascular chronic disease management.
• An innovative mindset, a passion for 1:1 patient counseling and driving behavior change, and excellent relationship building and communication skills are necessary.
• Should be able to effectively communicate across various mediums and be comfortable working in an agile start-up environment.
• Occasional travel for team meetings is also required
• Participate in the design and launch of Our Company’s comprehensive, evidence-based virtual-first cardiac recovery program
• Provide virtual psychotherapy (45-50 min per session) to a panel of patients
• Evaluate patient symptoms and risk factors to determine diagnoses and develop individualized treatment plans
• Ensure that treatment plans are supported by standard practices and/or current literature
• Follow plan of care to accomplish functions within established policies, procedures and scope of practice Re-evaluate treatment plans, as needed, to ensure safety and maximum patient benefit
• Promote patient engagement, and successfully achieve patient and program goals
• Make independent judgments and change to treatment according to professional and regulatory standards
• Counsel patients using behavioral change techniques (ex. mindfulness, motivational interviewing) • Communicate effectively across Our Company’s integrated clinical team and our partner health systems, escalating unusual and emergent situations
• Provide complete, accurate and timely documentation of patient sessions
