Position Focus:
The RN will actively support the patient care activities of physicians, advanced practice providers, social workers and other team members providing services to Pediatric Diabetes patients. Answer all routine diabetes patient inquiries made by telephone or MyChart messages, including but not limited to those concerning their disease, symptoms, treatment plans or medications. Answer all urgent, diabetes-related calls made during normal business hours and assist the patient by assessing the situation, recording patient data, providing recommendations to the patient, and supporting provider efforts to develop plans of care when necessary. Enter and pend prescription refills and other patient orders in accordance with office protocols. Serve as a source of knowledge for faculty and staff. Assess urgency of patient care needs and provide guidance on clinical scheduling. Obtain prior authorization for medications and devices. Review home care orders for provider signature, completeness and accuracy. Interact with the home care agencies to provide complete instructions and verify accuracy of orders per review of the medical chart, discussions with provider and working knowledge of the specialty treatment plans. Follow-up on patient lab results and work with clinical faculty to develop a follow-up plan that is then communicated to the patient/family.
Essential Duties
1. Performs physical assessments of patients, including triage duties and advises on treatment or course of action by having knowledge of nursing care standards and principles. 2. Develops, implements, and manages nursing care plans in collaboration with physicians. 3. Interviews patients and records medical history to establish data for treatment plan. 4. Plans and coordinates the admission and discharge of all patients. 5. Instructs patients and families in proper treatment and advises patients on health maintenance and disease prevention. 6. Performs patient care procedures by following appropriate policy and procedure. 7. Records patients’ medical histories and symptoms, operates medical machinery, administers treatment and medications, and coordinates patient follow-up and rehabilitation; assures all procedures for maintaining patient confidentiality are maintained per HIPAA guidelines. 8. Identifies education needed to assure competent delivery of patient care for oneself and others in the department. 9. May perform other duties as assigned.
Required Education and Experience
Graduation from accredited school of nursing.

Outstanding interpersonal skills. Excellent communication skills, telephone skills and writing skills. Demonstrated ability to work compassionately with patients and parents, and work collaboratively as a team member within the section.

Detail oriented, ability to multitask and prioritize work. Demonstrated ability to switch between tasks, maintaining focus.
Required Skill/Ability 3:
Demonstrated ability in pediatric medicine.
Required Skill/Ability 4:
Demonstrated computer proficiency, including with EMR.

Demonstrated ability in patient and family education.
Required Licenses or Certifications
Licensure as Registered Nurse in the State of Connecticut or Connecticut license eligible. Current CPR certification.
Preferred Licenses or Certifications:
Licensure as a Registered Nurse in the State of Connecticut.
