We have an opening for a Diabetic Nurse Educator. The ideal candidate is passionate about patients and their loved ones, and is committed to the advancement of community health and medicine. This role requires both expertise and an excellent cultural fit. Great communication skills, both verbal and written, as well as the ability to inspire and coach individuals and families are necessary.
The Diabetic Nurse Educator is responsible for:
• Providing diabetes education to newly diagnosed diabetic patients, as well as patients who have managed diabetes for years
• Assisting with diabetic transitions of care between inpatient and outpatient settings
• Completing needs assessment for diabetic patients
• Collaborating with providers to develop and implement individualized care plans for diabetic patients
• Educating patients about blood sugar assessment and management strategies
• Working with patients to create goals for care
• Following up with patients to assist them in meeting their diabetic care-related goals
• Maintaining a current knowledge base about best and next practices in diabetes care
• Participating in multidisciplinary care as appropriate
• Documenting care plans and progress in the medical record
Requirements:
• RN, licensed to practice in the State of Maine
• 2 years of relevant clinical experience (preferably with diabetes care and management)
• Certified Diabetes Educator, or ability to obtain certification within 4 years of hire
