What are the responsibilities and job description for the Visiting Nurse position at Your Health?
We are rapidly growing and seeking RN's and LPN's to service our patients throughout the Conway area. Visiting Nurses will service home and/or Assisted Living Facility patients in care coordination with the provider within their assigned service area(s).
This is a salary-based position. It is a full-time 8-hr position (8:00AM-5:00PM) (Monday-Friday).

The Following Service Areas Are Currently Available
- Conway
- Assisting providers with any medical needs (i.e., orders, labs, vaccination administration, etc.)
- Traveling to patient homes
- Assessments
- Assessing patients with any acute needs and facilitating a visit with the assigned Provider.
- Skilled Nursing
- Coordinating and Facilitating Telehealth Visits with Providers (including specialty visits)
- Medication Reconciliations
- Chart Documentation
- Chart Audit Reviews/Updates
- Addressing Quality Measures
- Addressing Risk Factors
- Enrolling eligible patients into Chronic Care Management
- Capturing Chronic Care Management
- Strong Communication with assigned Care Team.
- Promptly assisting your assigned Care Team with any medical needs (i.e., administrative duties, orders, vaccine administrations, etc.)
- Active LPN or RN license
- Excellent communication and interpersonal skills with the ability to build strong relationships at all levels of the organization.
- Strong problem-solving abilities
- Ability to work effectively under pressure.
