Position SummaryUnder the direct supervision Dover Site Manager and under a matrix system of supervision and guidance of the Director of Nursing, the Medical Van Medical Assistant is required to perform all duties of the medical assistant and will function as a resource person for the outreach team, provider and MAs who will be participating in events involving the Medical Van. The Medical Van medical assistant has demonstrated mastery of clinical skills and consistently works at the top of his/her scope of practice. In coordination with the Site Manager and Director of Nursing, and outreach team, the Medical Van CMA will ensure that the Medical Team is able to function as a true extension of the medical site by ensuring, a well-trained medical team and resources needed to provide primary medical care to patients unable to visit one of our permanent sites. Duties and ResponsibilitiesPerform duties and responsibilities as outlined in the CMA job description. Act as primary CMA for most Medical Van related events. Act as preceptor for new CMA Medical Van staff, providing orientation and training on clinical policies and procedures.Participate in Care Team meetings (i.e., huddles) to prepare for patient visits and anticipate needs.Pre-visit planning for patients on van schedule, which includes reviewing charts prior visit to identify what the patient will need during visit with the provider. This may involve contacting the patient, especially if they are a new patient will no record in the EMR.Follow up with patients regarding results or needed future appointments as requested by the providerReview scope of services to be provided at each Medical Van outing and ensure that appropriate supplies, medications, vaccines, equipment, and patient forms are available to meet the needs of each unique event.Inventory all consumable supplies and medications (including emergency medications) to be maintained on Medical Van and ensure that all supplies are replenished prior to any Scheduled or Emergency Van outings. Remove/replace those items close to expiration. Ensure all cardboard boxes are removed from van following re-stocking.Ensure medical equipment is properly inventoried with current labels with ISS and receives regular maintenance/calibration as per manufacturer’s guidelines and Zufall’s policies. Monitor the function of all clinical equipment on the van including but not limited to: refrigerators, vaccine coolers, glucometer, A1C machine, Hgb meter, exam tables, EKG machines, AED, Oxygen tank, etc. and arrange for repair/replacement as necessary. Notify manager of any concerns related to above equipment or new equipment needs. Monitor refrigerator temperature and document daily per Zufall Vaccine Maintenance Policy.Keep all required logs up to date, including and not limited to glucose, A1C, Hemoglobin, HIV, Strep, Pregnancy, Covid, AED, Oxygen tank, etc. Participate in collaborative initiatives and PDSA’s as directed by PCMH Coordinator, Director of Nursing, and Chief Medical OfficerEnsure that, at the end of the day, examination rooms and other clinical areas are clean. Ensure that tables and chairs are wiped down and all equipment and wall units are cleaned with antibacterial wipes.Perform other related duties as assigned by the PCMH Coordinator or Site Administrator and the Director of Nursing.Satisfactorily complete competency testing for the following, but not limited to: translation, vital signs and measurements, doing TB skin tests, all waived POC lab tests, infection prevention and control, sterilization process and instrument preparation, OSHA, immunization administration and reconciliation, standing orders, medical waste management, patient preparation for the visit, cultural competency and customer service.Attend appropriate outreach meetings to gather data necessary to ensure that all actual and potential needs of the clinicians and patients are met during an outreach van visit. Utilize content of meetings to effect change in both personal and organizational clinical practice.Identify and recommend solutions to clinical procedural and workflow related issues as they pertain to the Medical Van.Maintain confidentiality of all patient information.When not performing tasks specific to Medical Van, CMA will work at permanent sites as needed. Location may vary to meet the immediate needs of the organization. Knowledge, Skills and QualificationsBe fully bilingual in English and SpanishBe proficient in the use of computers, internet navigation and keyboardingHave excellent customer service and interpersonal skills, and be a team playerBe able to relate well to a diverse group of patients and staffDemonstrate self-motivation and leadership skills necessary to work offsite and without presence of direct supervision.Display positive attitude and act as positive agent for change in daily clinical practice as it applies to the Medical Van.Demonstrate consistent team-oriented work practices.Be able and willing to work on an as needed basis and in a flexible schedule to meet the operational needs of the Medical Van.Education, Training and ExperienceMinimum High School Diploma or GEDHave current certification as a Medical AssistantHave current certification for EKG, Phlebotomy and BLSMinimum 1 year experience as CMA at Zufall Health Center
