Located conveniently on Whitney Avenue in Hamden, we are a privately owned and operated 150 bed skilled nursing facility. We provide friendly and professional care in a well-appointed and safe environment for our short and long-term residents. Whitney Rehab specializes in sub-acute rehabilitation for orthopedic, pulmonary, stroke and cardiac patients. All rehabilitation programs are directed by a physician and completely individualized. All of our residents are treated with the utmost respect as they recover from their illness or injury. Our skilled multidisciplinary, professional team of Doctors, therapists, RNs, LPNs, APRNs, social services, and support staff bring the highest quality of care and extensive knowledge to our residents. Each resident has an individual plan of care that guides them in achieving their goals to better health. Our goal is to promptly help you get home. After our patients are discharged, they are welcome to return to us for outpatient rehabilitation where they will continue with the same therapists that helped them rehabilitate during their stay with us. Whitney Rehab is part of the Yale New Haven Health System Preferred Provider network. We are active members of the Hamden Chamber of Commerce and the New Haven Area Senior Network.
