Job Summary

• Plan, develop and implement activities programs in long-term care that assist to enhance the resident's highest practicable level of physical, mental, and psychosocial well-being.  
• Assess each resident in long term care and transitional care unit (TCU) for activity abilities and interests.
•  Provide an activities program designed to meet the individual needs and interests of each resident.  
• Develop an individual care plan as indicated from the assessment which will maintain quality of life for each resident.
•   Plan, develop, and implement a Day Program for the clients enrolled in Adult Day Services.  Interact with family, keeping family aware of progress and/or concerns. 
• Contact different departments for consultation to meet the client’s needs.  Communicate concerns and/or progress to those departments. 
• Responsible for; keeping up with new trends, attending professional meetings, maintaining liaison in the community designed to recruit groups and individuals, and establishing relationships with the various clergy who conduct religious services. 
• Interview, train, supervise and evaluate activity assistants and cosmetology employees. 

Education and Training:

• Preferred 2 years experience in a social or recreational program preferably in a long-term care setting within the last 5 years; or 1 year as a full-time activity coordinator in a resident activities program or equivalent; or has completed a training course approved by the State of South Dakota.
• GED or High School Diploma
• CNA certified or able to acquire certification.

Work Schedule

• Monday - Friday; 80 hours every 2 weeks
  • 8:00 am - 5:00 pm

Pay

The hourly range for this position is listed below. Actual hourly rate dependent upon experience. 

• $18.25 - $22.25 per hour
