Additionally, Activity Specialist performs a variety of tasks. Works under general supervision; typically reports to a supervisor or manager. A certain degree of creativity and latitude is required.     What is a Community Activity Specialist?
    • Responsible for on-site and community supervision, problem solving and advocacy on behalf of program participants.
    • Assist in creating a safe and enriching environment for clients of CW’s Day Service Option program by planning for and providing activities that are of interest to and within the capabilities of those served.
    • Transport program participants to events and community activities.
    • Implements goals and objectives and follows established program and behavior plans and document participant progress and any other required data.
    • Participates in required training sessions and maintains certifications, which include First Aid and CPR, PMT, Blood borne Pathogen Training, Public Service License (based upon position). Assists the Rehabilitation Counselor in developing Individualized Plans by providing information on participants’’ progress, outcomes and daily activities.
    • May assist in maintenance of consumer records.
    • Serves as CW, Inc. participant liaison to DDS staff persons, family members or residential providers, and advocate for program participants. May attend team meetings.
    • May have to attend to personal care issues of others.
    • Conducts all program activities in accordance with CARF, State and Federal Agencies (DDS, DMHAS, DCF and etc.) and CW employees.

    • Must be able to work as a member of a team.
    • Must have proficient verbal and written communication abilities.

    The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The incumbent will spend the time at work standing; walking and occasionally lifting up to 20 lbs. Specific vision abilities required by this job include close vision, and the ability to adjust focus. The incumbent is required to regularly talk or hear while fulfilling the duties of the job.
