Addiction Mental Health Assessor jobs in Vermont

Addiction Mental Health Assessor conducts initial and ongoing assessments of patients in Alcohol and Drug (AoD) addiction programs to determine mental health needs and establish care requirements. Provides initial treatment and refers clients to appropriate agencies/providers as needed. Being an Addiction Mental Health Assessor works with patients and families to develop appropriate chemical dependency service plans. Complies with standards in the area of evaluation, assessment, treatment, and service delivery to clients. Additionally, Addiction Mental Health Assessor requires certification as a Licensed Chemical Dependency Counselor (LCDC) or similar. Typically requires a bachelor's degree. May require a state license to practice. Typically reports to a supervisor or manager. Addiction Mental Health Assessor's years of experience requirement may be unspecified.   • Position: Preventing Youth Overdose (PYO) Grant Wellness Coach

    Summary of Position: As a member of the PYO Grant Wellness team, the Wellness Coach will mentor and provide supportive coaching to assist identified clients and groups in identifying and pursuing health and wellness goals.

    Specific Duties & Responsibilities:

    • Knowledge of Youth Substance Use Disorders (SUD) needs and services.
    • In conjunction with the Wellness grant team, create wellness activities on-site and in the community to support and promote healthy lifestyles.
    • Deliver wellness activities to identified clients and groups.
    • Promote consumer engagement and peer support programs in wellness activities.
    • Willingness to be trained or have knowledge of SUD in youth ages 10-26.
    • Provide support/coaching to reduce or eliminate behaviors that are considered high-risk.
    • Strong interpersonal skills.
    • Track outcomes and report to PYO primary investigator.
    • Organize quarterly health wellness workshops to promote and provide a platform for youth and families to interact and build supportive relationships.
    • Outreach to schools and the community to increase preventative information around risks of youth substance use.
    • Experience interfacing with multiple community partners.
    • Experience in coordinating care for clients and families.
    • Ability to work cooperatively and responsibly as a member of a team from diverse backgrounds.
    • Ability to work cooperatively and responsibly as a member of a team with colleagues, supervisors, agency staff, and collateral contacts.
    • Ability to tailor individual plans of care with clients that coordinate wellness, behavioral, and medical services.
    • Good understanding of child development, parent/child dynamics, and appropriate interventions with a heavy emphasis on wellness activities.
    • Ability to effectively, appropriately, and accurately communicate both orally and in writing.
    • Competence in the delivery of wellness activities.
    • Knowledge of recovery-based concepts helpful.
    • Ability to make critical decisions for self and clients.
    • Keep up-to-date clinical records.
    • Abide by confidentiality policies.
    • Follow supervision directives.
    • Collect evaluation data.
    • Perform other responsibilities as assigned.
     

    Training & Experience:

    Required:

    • Bachelors degree in a human services field, or health education, athletic training, or nutrition.
    • Knowledge regarding topics such as youth SUD and risks, recovery supports, relapse prevention tools.

    Desired:

    • Good communication skills.
    • Recovery Coach.
    • Good organizational skills.
    • Enthusiasm for living a wellness-focused lifestyle.
    • Knowledge of WCMHS overall organization.

    Additional Requirements: Due to the rural nature of our catchment area, a valid driver’s license, excellent driving record, and access to a safe, reliable, insured vehicle are required.
