Social Services Director Job Summary & Qualifications: • Collect all data used in comprehensive discharge plans, including the MDS and Care Plan for new admits • Assist new residents in adjusting to the facility. • Coordinate and Lead staff meetings and other family and group meetings. • Document and manage facility and other clinical progress notes as it relates to the individual care plans for residents • 1-3 years experience in social work, preferably in an LTC or SNF.
