Friends & Family Caregivers is a trusted provider of Indiana's Structured Family Caregiving program, offering financial, emotional, social and other supports to the live-in caregivers across southern Indiana.

Full-time care coaches maintain a caseload of 40-50 participants. Our current needs are in eastern Vanderburgh and some neighboring counties. Individuals interested in part-time positions are also encouraged to apply.

The Care Coach works together with the care team to assess participant program eligibility and fit, collaborates with clients and caregivers to implement care that maintains or improves their independence and well-being. Also provides on-going training, education and support by completing home visits, assessments and other assigned tasks.

Qualifications:

• Previous relevant experience, especially in home-and community-based settings with aging and/or disabled populations
• Great communication and assessment skills
• Strong organizational skills
• Ability to use, implement and train others to use program software
• Have and maintain a valid driver’s license and a reliable vehicle with auto insurance and the ability/willingness to travel as needed
• Have successfully completed orientation to the position

Responsibilities:

• Acts as the main point of contact for assigned caseload, providing ongoing education and support
• Maintains charting and other electronic documentation in company approved software
• Participates in care-coordination, including assessment and referral of program participants to appropriate community agencies and intra-agency programs
• Reviews, analyzes, and reports incident data to determine improvement opportunities and manage interventions
• Performs home visits and completes appropriate assessments, visit reports, etc.
• Prepares reports, case summaries, and other documents for supervisory review
• Assists with other responsibilities as assigned by supervisor
• Performs all duties in a professional, conscientious, and efficient manner and in compliance with Company policies and procedures

Pay: From $55,000.00 per year
