Full-Time Onsite Position

Overview

Our Award Winning Client, a nationally recognized healthcare provider, is seeking an RN Home Health Intake Coordinator to join their team in New Brunswick, NJ. This is a full-time onsite position. The ideal candidate will have experience in Community Health, with Home Health or Hospice experience preferred.

Responsibilities:

• Assess and evaluate patients' home health care needs, developing appropriate care plans
• Provide education and support to patients and families
• Coordinate referrals to home care services and hospice
• Collaborate with other healthcare providers to achieve optimal patient outcomes
• Document changes in patient condition and update care plans accordingly
• Observe and report significant patient conditions and reactions to supervisor or physician
• Communicate effectively with members of the healthcare team
• Use oral and written communication to convey pertinent information in a timely manner
• Maintain strict confidentiality of patient information

Required Qualifications:

• Associate degree or higher
• 2 years of experience in Community Health
• Valid Registered Nurse (RN) license in the state of New Jersey (NJ)
• Valid American Heart Association Basic Life Support (AHA BLS) certification

Preferred Qualifications:

• 1 years of experience in Home Health or Hospice
• Bilingual in Spanish
• RN License Number

Technical Skills:

• Proficient in Microsoft Office Suite
• Experience with electronic health record (EHR) systems
• Strong clinical assessment skills
• Excellent verbal and written communication skills
