OUR AWARD-WINNING CLIENT operates nationally and has garnered numerous accolades for excellence in senior healthcare. They are currently seeking a qualified Nurse Practitioner (NP) for a full-time position in Minnesota (MN).

JOB DESCRIPTION: Our client is in search of a skilled Nurse Practitioner (NP) with a Master's degree or higher and a valid NP license in Minnesota (MN). The successful candidate will provide direct patient care services within the Post-Acute Care setting.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

• Conduct Annual Wellness Visits and perform Annual History and Physicals as needed.
• Administer clinical interventional and episodic direct care visits, alongside appropriate re-evaluations.
• Evaluate and treat chronic, co-morbid illnesses as necessary.
• Monitor residents for changes in condition and respond accordingly.
• Review medications, orders, and diagnostic tests, coordinating care with collaborating physicians.
• Collaborate with other disciplines and maintain communication with families.
• Additional duties as assigned.

QUALIFICATIONS:

• Valid NP license in Minnesota (MN) and current board certification.
• Bachelor's and Master's Degree in Nursing from accredited institutions.
• Nursing experience required; NP experience preferred.
• Proficient in clinical NP critical thinking skills applicable to the elderly.
• Strong leadership, interpersonal, and problem-solving skills.
• Excellent written and oral communication skills.
• Punctuality, organizational skills, and ability to work within a team.

Pay: $110,000.00 - $120,000.00 per year
