We are searching for a Medical Records Specialist to perform diversified duties both within and in support of the VNA Medical Record Department. Answer telephone requests for patient information. Review patient record to determine if content is protected and if so, ensure patient authorization for approved release. Prepare and scan all discharged records daily. Perform audits and complete corrections of inaccurate data in data system. Determine physician and clinician record completion status and provide him/her with notification of need for chart completion. Serve as a resource to physicians and clinicians by answering their questions related to missing medical record documentation. Ensure compliance with applicable hospital policies including HIPAA and General Laws of Massachusetts with regard to the release and use of confidential patient information and when appropriate, request the patient complete an authorization form to request copies of medical records
