JOB TITLE: Support Specialist, In-Home
SUPERVISED BY: This position reports to the Program Director, Residential or his/her designed, and will adhere to all policies and procedures of St. John’s Community Services.
Job Summary: Provides support to individuals with disabilities in their natural homes to develop and/or maintain skills that assist the individuals to be self-sustaining in that environment. Skills and supports provided should enhance the quality of the individual’s life and support the person in performing his/her own personal care, household tasks, daily routines and community integration activities.
Essential Functions:

Provide training and support in nutrition, personal hygiene, health care, appropriate dress, communication, interpersonal relationships, home management and use of leisure time.

Accompany individual to and facilitate participation in visits for medical care, therapies, personal shopping, recreation and other community activities.

Provide training or assistance in meal preparation, laundry, housekeeping, and simple home repair.
- Provide training and support in the areas of social, emotional physical and intellectual development.

Support the individual in working on the goals they have selected for themselves.

Document the specific support provided to the individual per the Individual Service Plan (ISP) and all progress made toward those identified goals.

Maintain and submit all required documentation (progress notes, incident reports, etc.) on specified due date.

Attend and participate in all relevant agency events, workshops, training and meetings; and

Performs other job related duties as assigned.
Required Knowledge, Skills, and Abilities:
- Ability to gain working knowledge of St. John’s Community Services policies and procedures;

Ability to act as a representative of St. John’s Community Services to the public;

Ability to effectively communicate and coordinate with other staff members and individuals from diverse backgrounds;

Ability to pass a criminal background check;

Knowledge of acceptable food preparation and safety practices and procedures; and
-
Skilled in the operation of relevant computer systems, including hardware and software, and office machines.
Education and Experience:
- High school diploma or G.E.D. required

Two or more years related experience working with persons with disabilities preferred; experience in community based programs a plus;

Must meet current SJCS and funding jurisdiction requirements for criminal background/records check.

Must meet current funding jurisdiction requirements for valid health certificate.
- Valid state-issued driver’s license with current driving record in good standing required.
